Thursday 11/16 Pre-Surgery Nutrition Class

Getting ready for bariatric surgery?

If you have been instructed to complete a pre surgery nutrition visit, this is the correct class for you!

If you are required to do additional visits (3, 6, 12), you will be sent instructions later on how to schedule those visits.  Repeating this class will not count towards your additional visits. 

A valid credit card is required at the time of booking to hold your space.

You are not charged anything at this time.  

By providing this payment information, you are agreeing that it may be used to collect any balance due if insurance is being filed. 

CASH PAY patients will be charged AFTER completion of the class.  The fee is $75 for cash pay patients.

If your physician is paying for this class, don't worry, we already know that. :)
