Video for Insured Patients: Preparing for Surgery

This class will satisfy your insurance or physician requirement for a single pre-surgery nutrition visit. 

Cost: $25 deposit

A $25 deposit will be collected in advance of the initial appointment. This fee:

• Will be credited toward your balance due after insurance processes your claim.
• Will be refunded in full if insurance pays the total cost of your visit and no patient responsibility remains.
• Is not a barrier to receiving care, and will be waived in hardship cases upon written request and approval.

This fee is not an additional charge and is intended to secure your appointment time and reduce no-shows.

You will be texted and emailed the link to the video information.

If you are required to do more than one visit, you will be sent instructions on how to schedule those visits.  Repeating this class will not count towards your additional visits.
